Prosthetic treatment of a boy with Hypohidrotic Ectodermal Dysplasia and Anodontia in the lower jaw – a case report from childhood to adulthood

Aim

The aim of this presentation is to describe the prosthodontic habilitation over a period of 20 years of a boy with hypohidrotic ectodermal dysplasia and agenesis of 22 permanent teeth. He also had symptoms of sparse hair, dry thin skin, reduced sweating capacity, and reduced salivary secretion rates.

Material and methods

To establish a treatment plan for this patient, a multidisciplinary team was formed comprising the specialities of paediatric dentistry, oral surgery, orthodontics, and prosthetic dentistry. The boy was one of the first children to be treated with dental implants at the age of 6. Two cylinder titanium implants (Brånemark System®) were installed under general anaesthesia in the canine regions of the anodontic lower jaw. An overdenture was fitted until the boy started school at the age of 7. A removable partial denture was then made for the upper jaw. The dentures were relined and remade several times during the years the patient grew up. When the patient was 19, two additional implants were placed in the lower jaw and a fixed prosthesis was made. After orthodontic treatment at the age of 22, the patient received two tooth-supported fixed partial dentures in the upper jaw allowing a small diastema in the midline.

Results

All therapy decisions were made in close cooperation with the boy and his family. The treatments performed during the patient's childhood and adulthood restored function and appearance in a very favourable way. All oral tissues are healthy, and because maintenance care was meticulous, hyposalivation has not caused any signs of disease.

Conclusions

The early placement of implants in the canine regions of the lower jaw created good prerequisites for a lower overdenture during childhood and adolescence. The same implants are still in use in good positions in a fixed partial denture with two additional implants.
